EndnoteWeb + CWYW + Chicago 16th Footnote + "Web Page" = wrong footnote format?

I am just not beginning to use EndnoteWeb + CWYW and already the results are disappointing. I just manually entered an online document from an online database, and when I insert the citation into a MS Word (2013) footnote, I get what appears to be only an inline citation. Where I should be getting something like

“McDonald’s Happy Meal Toy Safety Facts,” McDonald’s Corporation, accessed July 19, 2008, http://www.mcdonalds.com/corp/about/factsheets.html

I get

“McDonald’s Happy Meal Toy Safety Facts,” http://www.mcdonalds.com/corp/about/factsheets.html

Going on a hunch, it appears to be inserting the citation in an in-line format rather than footnote formet (which includes access date, etc. I have filled in the fields Title, Year, Series Editor, Series Title, Access Year, Access Date, and URL (all of which need to be included in this particular citation), and have selected “Web Page” as the reference type.

Can anyone help? Is this an Endnote issue? Or is it perhaps something to do with Word?

PS The bibliography is formatted the exact same way as the footnote, with no access date, editor, etc.
