What version of endnote are you using?
Make a copy of the document and convert to temporary citations. Are they correct or moved too? If correct, make sure there isn’t one still as a field or junk, which may be corrupted. if you find one, delete it and reinsert. Now format and see if it is fixed. – If this doesn’t work, call the endnote tech support, as they have tools that can be used to restore the appropriate citations.
