I'm interested in having endnote place my in-text citations as superscript numbers using the JAMA citation style. Currently, my in text citations are inserted as regular text. 

Does anyone have any advice?

The formatting issue that you're describing is known to impact the Cite While You Write plug-in that Apple developed for Pages 6:

If you've been making Time Machine backups of your hard drive, and one of these images contain a backup of the Pages 5 app file, I'd suggest reverting to that previous version of Pages to resolve this problematic behavior.
