I am using EN 21.5 on macOS Sonoma. Is there a way to change the appearance of an individual reference? I am happy to have Author1 et al. XXXX everywhere except in one place where there are two joint first authors.

Yes, assuming you asking about the way it appears in the text as a citation? The best way is to right click (or whatever the equivalent is on the Mac) and choose to edit the citation to “exclude author” and then repeat, and this time choose “more” and add the authors as you want them to appear to prefix field including punctuation and space to separate it from the year field.
