I’m new here so sorry if this has been asked before! Is there a way to create a custom group in EndNote X4 on the Mac that lists the citations currently not used in a Word document? I have the automatic groups that tell me my total citations (All References), and those used in a particular document (currently Literature Review.docx) but I would like to see those not used in the Word document just in case I’ve missed one or two that would be useful to have.

What you want to accomplish is possible using the “Create from Groups” function but it is admittedly a bit cumbersome. You would first need to re-create your “All References” and “Literature Review.docx” auto-groups as custom groups and then use the “Create from Groups” function to create a group that listed all references NOT in your Literature Review.docx group.
