multple authors in citation where et al should appear

I’m on EN X2 on Mac v 10.4.11.  Second try getting proper formatting of citations for journal Tox Sci.  I tried using a new .ens file sent to me.  This helped a lot, but some citations retain multiple authors, though I have unformatted and reformatted, even deleted and re-downloaded the references.  

I checked the formatting of the .ens, andit says "if more than 3 authors, list the first and follow with et al.’ as it should.  But it does not do it consistently.

 Thoughts?
