multple authors in citation where et al should appear

I’m on EN X2 on Mac v 10.4.11.  Second try getting proper formatting of citations for journal Tox Sci.  I tried using a new .ens file sent to me.  This helped a lot, but some citations retain multiple authors, though I have unformatted and reformatted, even deleted and re-downloaded the references.  

I checked the formatting of the .ens, andit says "if more than 3 authors, list the first and follow with et al.’ as it should.  But it does not do it consistently.

 Thoughts?

Did you check the settings in both the first appearance and the second?  Also, what are the ambiguous authorship settings? 

You can upload the style you are using and we can take a look at it?
