Upper case characters in titles of journal articles

I want to use the ‘sentence’ style for titles of journal articles, i.e. no caps except after full stop.

But how do I make sure that England appears as England in reference and not as england or that NVQ does not appear as nvq.  I guess that I can take the whole file and save as rich text and then edit manually.  Surely, however, there is ay that I can retain ‘sentence style’ as universal style but overide where grammatically necessary?

Edit>preferences>change case provides you with a place to put those sacrosanct words.
