This journal puts the first author in each bibliography entry in bold. I can’t see a way to do this within the style, without making all the authors bold. Can anyone help?

In my experience if a specific publisher has unusual formating requirements like this, they can deal with it in house. Neither Endnote nor Endnote Web have the flexibility to automatically bold the first and not subsequent authors. And when I go to Author instructions, there is no mention of this? http://group.bmj.com/products/journals/instructions-for-authors/formatting#references

Thanks for that. There is currently a mismatch between the instructions to authors and the actual reference format in the journal…

Which probably means they deal with it in-house with their publication processing. I would just focus on the content, and let them worry about the bolding of authors.
