Endnote-Pages installation

I just downloaded and installed Endnote X5 in Lion for use with Pages 9.  I also installed the Pages plug-in

for Endnote.  Two problems:

  1. There is nothing in the Pages Menu to indicate that the plug-in is working/active.  e.g. Insert>citation

  2. I get the message ‘The document “My EndNote Library.enl” could not be opened’  after I open EndNote.

It is probably something very simple, but it is definitely not obvious
