output style/cite while you write problem

I am using Endnote X3 for Windows XP.  I have used the programme with no problem recently, but now my “author-date” output does not incert my citation in the text at all and puts all fields, unformatted (just as I see them in Endnote) as bibliography.  Sometimes it puts in more than one. 

I have downloaded all the styles again, and have tried to create a custom style, but it does the same thing.

Just with that specific document?  Ensure that when you reformat the paper (using the reformat bibliography word/endnote menu choice or toolbar item) that the style selected for that manuscript is the style you think you are using.  You can change it from a drop-down (although you may have to browse the style folder) and then “okay”.
