I’m working on a multiple-chapter Word file. I’m using the “numbered style” for citations and interestingly the first citation is listed as #2 [2] while citation #1 [1] is inserted somewhere in the middle of the document. Does anyone have any idea how I can fix this so the first citation number would appear as [1].

See http://endnote.com/kb/82359 for a discussion of the likely cause (citation inserted into a textbox usually associated with a figure), but view this recent solution to the problem which is a better fix than that oulined in the first link.
In addition, this fix also applies to other word related issues where figure titles are not recognized by Word to include in a table of figures, for example. Word has real problems recognizing fields in textboxes. It always has.
